    Saving Time in Portfolio Optimization on Financial Markets

    The time management is important part for tasks in real-time operation of systems, automation systems, optimization in complex system, taking explicit consideration in time constraints, scheduling of tasks and operations, making with incomplete data, and time management in different practical cases. The limit in time for taking appropriate decisions for management and control is a strong constraint for the implementation of autonomic functionalities as self-configuration, self-optimization, self-healing, self-protection in computer systems, transportation systems, and distributed systems. Time is an important and expensive resource. The time management in financial domain is a prerequisite for high competitiveness and an increase in the quality of the investment activities. It is the popular phrase that time is money, and particularly, the portfolio optimization targets its implementation in real cases. This research targets the identification of portfolio parameters, which are strongly influenced by time. We restrict our considerations only on portfolio optimization task, and we identify cases, which are strongly influenced by time constraints. Thus, the portfolio optimization problem is discussed on position how the time can influence the portfolio characteristics and solutions. This chapter starts with the description of the object portfolio management, which provides the cases where time in explicit way influences the portfolio problem

    Influence of Yoga Practices on Stress Coping Strategies

    Stress coping strategies are used every day. By choosing a different coping style, people seek solution to a complicated task or situation. One of the reliable forms of motor activity, which influences favorably the development of valuable psychic and physical qualities, is yoga. The aim of the research is to determine the positive effect of yoga practices on stress coping strategies. The research was done among 76 women - 42 of them practice yoga in different clubs of the Bulgarian Yoga Federation and yoga students at NSA “Vassil Levski” – experimental group; and control group consisting of 34 women not engaged in any kind of sport or motor activity. The stress coping strategies were evaluated with the questionnaire СОРЕ-1, (Carver, C, & Scheier, M. 1985), adapted for Bulgarian conditions by A. Rusinova-Hristova and G. Karastoyanov (2000). Results show that the first seven stress coping strategies have approximately equal values with the control group. The passive strategies, related to emotion-focused coping with the problem, are dominant. In this case, the structure of the stress coping strategies with the women not engaged in sport is more imperfect than the one with the women practicing yoga.  The arrangement of the leading stress coping strategies, as well as the reliable differences with six of the strategies (43% of all 14 strategies) in favor of the EG is indicative of more harmonious and more perfect structure of the preferred stress coping strategies with women practicing yoga. Conclusion: The leading stress coping strategies with women not engaged in sport are passive. They experience greater difficulty in solving their problems in stressful situations. The leading strategies with women practicing yoga are active. The researched individuals react immediately in stressful situations, concentrate on the problem and actively cope with it.     Summary: The carcinoma of mammary gland is the most frequent female malignant disease in Bulgaria and in most of the developed countries. Its share is about 25% of all malignant diseases and it takes place second among the mortality reasons due to oncological diseases. The cancer disease inevitably leaves a trace in women and changes their quality of life, influences their psychosocial status, causes anxiety and distress and changes their views on life and the future. All the spheres building the concept „quality of life` are influenced in different ways by the dynamics of emotionality and experience. The current research study examines the correlation between subjectively estimated anxiety which is proved by tools and psychosocial functioning in women with breast cancer

    A real – life observational pilot study to evaluate the effects of two-week treatment with montelukast in patients with chronic cough

    BACKGROUND: Different conditions make the proximal airways susceptible to tussigenic stimuli in the chronic cough (CC) syndrome. Leukotrienes can be implicated in the inflammatory mechanism at play in it. Montelukast is a selective cysteinyl-leukotriene receptor antagonist with proven effectiveness in patients with asthma. The aim of our real-life pilot study was to use montelukast to relieve cough symptoms in patients with CC allegedly due to the two frequent causes other than asthma – upper airway cough syndrome and gastroesophageal reflux (GER). METHODS: 14 consecutive patients with CC were evaluated before and after 2 weeks of treatment with montelukast 10 mg daily. Cough was assessed by validated cough questionnaire. Questionnaires regarding the presence of gastroesophageal reflux were also completed. Cough reflex sensitivity to incremental doubling concentrations of citric acid and capsaicin was measured. Lung function, airway hyperresponsiveness and exhaled breath temperature (EBT), a non-invasive marker of lower airway inflammation, were evaluated to exclude asthma as an underlying cause. Thorough upper-airway examination was also conducted. Cell counts, eosinophil cationic protein (ECP), lactoferrin, myeloperoxidase (MPO) were determined in blood to assess systemic inflammation. RESULTS: Discomfort due to cough was significantly reduced after treatment (P < 0.001). Cough threshold for capsaicin increased significantly (P = 0.001) but not for citric acid. The values of lactoferrin and ECP were significantly reduced, but those of MPO rose. EBT and pulmonary function were not significantly affected by the treatment. CONCLUSION: Patients with CC due to upper airway cough syndrome or gastroesophageal reflux (GER) but not asthma reported significant relief of their symptoms after two weeks of treatment with montelukast. ECP, lactoferrin, MPO altered significantly, highlighting their role in the pathological mechanisms in CC. Clinical trial ID at Clinicaltrials.gov is NCT01754220

    Laparoscopic Treatment Of High Sigmoidovaginal Fistula. Case Report

    Rectovaginal and/or colovaginal fistulas are difficult-to-treat conditions that can cause vaginitis, abnormal flatulence through the vagina, skin excoriations, and more. Depending on the type of fistula, they can be rectovaginal, anovaginal, colovaginal, enterovaginal, vesicovaginal, ureterovaginal, urethrovaginal, with the most common being vesicovaginal and rectovaginal. In most cases, these conditions present a challenge and require a comprehensive diagnostic approach and treatment. We present a clinical case of a 73-year-old patient manifesting with flatulence through the vagina. During hospitalization, a high sigmoidovaginal fistula was diagnosed. The patient has a history of hysterectomy 17 years ago, due to myomatous uterus. Subsequently, she had three operations for postoperative hernia, two of which involved the placement of synthetic mesh. During the diagnostic plan, diverticulosis of the colon was also detected. This was observed as probable etiological cause for the formation of the fistula. The patient had comorbidities: arterial hypertension and severe obesity (Grade III). A laparoscopic disconnection of the fistula was performed, followed by laparoscopic suturing of the vagina and sigmoid colon, with subsequent omentoplasty. After an uncomplicated postoperative period, the patient was discharged on the 5th postoperative day, fully mobilized, with restored gas and feculent passage. There are few cases of high sigmoidoaginal fistulas described in the medical literature. The treatment in such patients is still unclear and subject to discussion. With the advancement of minimally invasive techniques in medicine, the laparoscopic surgical approach is a suitable option for treatment, but long-term follow-up and in-depth analyses are necessary

    Late Diagnosis of Urinary Peritonitis Due to a Lesion of the Left Ureter During Laparoscopic Anterior Resection of the Rectum—Laparoscopic Solution. Clinical Case and Literature Review

    Ureteral lesions are a rare complication of colorectal surgery. This type of surgical procedure is the second most common cause of such complications, second only to gynecological operations, which account for around 50%. According to the localization of the lesion, they аre grouped into three types: injury to the proximal, middle, and distal part of the ureter, with the latter being the most common—around 80–90%. Most of the lesions are not recognized intraoperatively and sometimes the diagnosis can be delayed significantly. The time of diagnosis and the localization of the injury are crucial to the choice of treatment. We present a case of a patient, with a late diagnosis of lesion of the left ureter after the laparoscopic anterior resection of the rectum, which was treated with a laparoscopic uretero-ureteral anastomosis with simultaneous double-J catheterization. We also present a short literature review on the subject

    Post COVID-19 Conditions and the Cardiovascular System

    One out of four patients affected by COVID-19 will experience persistent (>3-4 weeks) signs and symptoms (Post COVID-19 conditions or Post-Acute Sequelae of SARS-CoV-2 – PASC) and this fact will have a major significance for the healthcare and economic systems in the upcoming years. The cardiovascular system is one of the key targets for the Post COVID-19 syndrome, given the pathogenesis of the virus and prevalence of ACE-2 receptors. According to our initial personal experience via the campaign “Life after COVID” of the Bulgarian Cardiac Institute, a substantial proportion of patients having suffered from COVID-19 develop long-term cardiovascular consequences. They could range from rhythm disorder and blood pressure variation, through impairment of myocardial mechanics and heart failure, and to acute vascular manifestations of Post COVID-19 conditions, such as acute coronary syndrome, acute pulmonary embolism, and acute limb ischemia. These cardiovascular complications require special and dedicated medical attention, and we could share our personal experience on the matter
